New options for enteral nutrition in patients with severe acute pancreatitis




Severe acute pancreatitis (SAP) requires an adequate nutritional support. Enteral nutrition (EN) should be preferred to total parenteral nutrition in patients with SAP, as it is associated with reduced mortality and complications. However, in clinical practice EN is employed far less frequently than it should. The main obstacle to EN diffusion is that it is considered complicated, as to ensure full pancreatic rest, nutrition tubes should be placed in the jejunum, requiring often troublesome procedures. In the past few years, it has been proposed that EN through nasogastric (NG) tubes may be a simple, safe and equally valid alternative to nasojejunal tubes.


A research article published on August 7, 2010 in the World Journal of Gastroenterology addresses this question. The authors speculated that a pragmatic possibility in real-world clinical practice would be to employ NG feeding whenever tube migration to the jejunum of bedside inserted feeding tubes does not occur spontaneously. They therefore aimed at assessing the rate of spontaneous distal migration of EN tubes in patients with predicted SAP, to identify possible factors associated with it, and to compare the safety and tolerability of EN with an elemental formula in patients who started nutrition with a "proximal", NG or a "distal", naso-intestinal tube, depending on the success of spontaneous tube migration.


This is the first study of its kind observing the outcome of EN in SAP patients in a "real world" clinical setting, with the study protocol driven by the need to have more solid grounds in making clinical decisions about everyday medical care circumstances. Both the proximal and the distal enteral approaches resulted to be feasible, safe and effective in most patients. This issue has a relevant impact on everyday clinical practice as the main limit to EN usage in AP is the technical difficulty in obtaining small bowel access.

A linebacker, like any other position on the gridiron, requires sport specific weight training to become the best at his position. Here, we will only cover the weight lifting aspect of linebacker training, set up on a 5 day split to build strength and power. Using this weight lifting routine, you should see improvements in speed, vertical leap, explosive power through tackles and a better ability to get off blocks.

Day 1 - Posterior Chain Work
This day is to build up the explosive power in your p-chain to make you run faster, jump high, and explode through tackles. The muscles in your p-chain include the lower back, glutes, hamstrings and calves.
Cleans 5x3
Dead lift 5x5
Stiff leg dead lift 3x8
Hypers 3x8
Calf raises 3x20

Day 2 - Shoulders and Chest
Building up power in these muscles will help you to get off blocks and bull rush, along with shoving people around. Focus not on the weight, but moving the weight quickly and explosively. Imagine each time you perform a lift that you are hitting another player.

Bench press 5x5
Dumbbell Snatch 5x5
Dumbbell lateral raise 3x8
Cuban rotations 3x10

Day 3 - Back and Pulls
These lifts will balance out the muscle you build in your chest and shoulders. By building up your traps, lats and middle back you will be better at pulling people around and wrapping up tackles.

Day 4 - Quads, hip flexors and abs
This day is all about leg drive. Leg drive will make you faster and more explosive through tackles. Side lunges will help with deceleration and change of direction. Leg raises build strong hip flexors to help you pick up your legs faster when you run. As you should know, foot speed and leg drive are the key components in speed.

Ab work with weight 3x8

Day 5 - Arms and Neck
Strong arms will assist you in all of your work as a linebacker. Strong triceps will boost your pushing movements and strong biceps will improve your pulls. Strong forearms are crucial to grip strength and neck work will help prevent neck injuries and keep you solid in tackling.

Yankees What Happened To Our Don?







Joe Torre is undoubtedly one of the most successful managers in sports history. Starting as a manager for the Yankees in 1996, Torre went on to manage the team for 12 straight years, bringing them to the playoffs in each year. Also add in four championships.

At the end of the 2007 season, Joe Girardi took over as the Yankees manager, and Torre went on to manage the Dodgers. Three years later, he has announced that he will be stepping down as manager of the Dodgers in 2011, and will hand over the job to Don Mattingly.

Here are the top five lowest points of Joe Torre's illustrious career.



Sports Illustrated's Jon Heyman examined three of the winter's open managerial posts (the Cubs, Mariners, and the potentially open Dodgers job) and passed along some info on who might be in the dugouts for those clubs come Opening Day 2011...


  • Chicago. Cubs legend Ryne Sandberg "has been seen by everyone as the most logical choice."  Heyman cites Joe Girardi as the second-most likely option to manage the Cubs in 2011, though he expects Girardi to remain in New York.  Interim manager Mike Quade is "something of a longshot" and Heyman mentions Bobby Valentine's name due to Valentine's track record of working with young talent.  Heyman's fourth-most likely Cubs manager is Tony La Russa, and while that hiring might equally horrify both Cubs and Cardinals fans, Heyman only opines that La Russa would "be an interesting choice," not one that's a distinct possibility.  

  • Los Angeles.  "According to some in the know," Tim Wallach will be the next L.A. manager.  Wallach is a former Dodgers hitting coach and has managed their Triple-A squad for the past two seasons.  Los Angeles would save some money by hiring Wallach, though Heyman writes that the franchise has (and will) cut enough payroll to make signing a big-name manager feasible if the team wants to go that route.  The intended Joe Torre/Don Mattingly succession may be no more given how that plan was the one favored by Jamie McCourt, not current sole owner Frank McCourt.  Heyman ranks Mattingly third on his list of likely Dodger managers behind both Wallach and Dusty Baker, as "there are unsubstantiated whispers" that L.A. will make a play for Baker if he doesn't re-sign with Cincinnati.  Of course, this could all be moot if Torre decides to return for another season, though Heyman feels Torre is probably done in Los Angeles. 

  • Seattle. Valentine appears again on the list of Mariners candidates, sandwiched between Heyman top choice Ted Simmons and No. 3 choice Willie Randolph.  Simmons, the San Diego bench coach, has never been a manager, though he has a lengthy front office resume.  Both Simmons and Randolph have worked with Mariners GM Jack Zduriencik before, and Randolph is also helped by the fact that Zduriencik told Heyman that "big league managerial experience will weigh heavily" in his decision.  That said, Zduriencik also noted that he's just starting to explore a list of around 20 candidates.

School: Back to school shopping ripoffs?

This week in my tiny town's back-to-school sale flyer, I am urged to buy Winchester Ammo at 5.99 a box for... The backpack? An art project? To trade for gum cards? This 'Back to School Sale' flyer goes on to try to convince me that every child needs a new 39.99 vacuum cleaner to drag to class, along with some name-brand frequent heartburn pills to munch on if they get sick after plugging in their new school days coffee maker.

A brief history of the back-to-school sale:

Back-to-school sale ads have evolved over the years from their rough beginning near 1904 when child labor laws started to reform. Prior to 1904, children were used as labor on farms, in factories, and around the house. Schooling was a rich-kid sport, and costs of items related to school were priced accordingly. Education was considered a luxury by both children and adults; clothing was advertised to reflect this honor of being educated.

Sears 1897 catalog back-to-school sale selections included: boy's double-breasted suit with two pair of knee-pants and stylish hat for 2.25. Proper "school" shoes in white with long pointed toes at 1.28. Suspenders for 2.00. Nicely ruffled and pleated boy's shirt for 1.10 and knee sox for .67. One suit of clothing for a school boy cost over 7.30 plus shipping, while average incomes for year 1897 were only at about 415.00 per year.

Back-to-school sale ads evolved between 1904 and 1936 to include school related items available to all social class budgets. In 1904, a National Child Labor committee was formed for the purpose of attempting to convince the reluctant public that children should be in school and not be out working to contribute to sparse family income. Schooling was still an honor, but back-to-school sales were now being influenced in part by economics and the Great Depression. Children without jobs could eat free if they went to school.

Sears 1923 catalog back-to-school sale is mid-way through the government fight to get children out of labor and into a school classroom. A boy's dark blue serge suit with one pair of knee britches and hat "for school wear" is priced at 4.25, with shoes and accessories 8.40. Average income for the year 1923 was 2,126.00 per year; with rent at an average of 18.00 a month, this left proper school clothing in the budget of some lesser-than-rich American homes.

Near 1923, we also saw the birth of some of our common school supplies today at Sears: school bags with straps and leather "good quality" handles 1.23. 6pc pencil paper tablets with colorful picture covers with 40 sheets of paper each for .25. Two boxes of Crayola wax crayons with 16 colors for .37. The 1923 Sears back-to-school sale also offers an item called "new idea" - a square black metal lunchbox with a thermos bottle inside for just 2.43.

In 1936 it finally became Federal Law that all children should be in classrooms and not the factory. By 1937, manufacturers everywhere were basing ads towards back-to-school sales on all kinds of products with a children's theme in the advertising. The theory was that the children would talk in class, and go home to convince their parents to buy these newly themed items. An example of a 1937 children's ad is seen in the Post Toasties Corn Flake ad for radio play from the AGMA approved grocery ad campaign book for that year:

"The better corn flakes. Mickey Mouse toys on every package. Delicious food for the family - toys for the children. Stays crisp longer in milk or cream. Crisp and delicious. In the big red and yellow box. Factory-fresh to you. The double crisp corn flakes. Made from the tender hearts of corn."

The beginning advertising campaigns toward children worked. By the mid-1940 era no child wanted to be seen on campus without their Roy Rogers lunchbox and book bag in tow.

During the 1940's schools moved away from the formal dress codes due to the new understanding that all children could not afford such luxuries. Mother's started stocking up on starch in back-to-school sale ads to iron creases into little boy functional trousers to show traditional respect toward schooling.

As televisions moved into every home in America during the 1950's and 1960's, so did the need to keep up with the newest items being offered in back-to-school sale ads. Dressing children for education was no longer about honoring the luxury of education; it was now a consumer fashion war that has led us into today's shop-frenzied standards on the back-to-school sale ads.

While I still have no clue what the little store here in my town was thinking about when they ran Winchester Ammo at 5.99 in their back-to-school sale ad this year, I do know that I do not enjoy back-to-school sale ads any more. They seem hollow in comparison to the old-time back-to-school sale ad definition of feeling honored to be in class.
